Poor Big Brother! All work and no play made him a sick boy. Host Toni Gonzaga blurted out the news to the housemates during the live nomination night: Big Brother was sent to the hospital to ensure his health condition. After days of struggling with cough and whatnots, he collapsed while doing his rounds of daily confessions with the housemates. The news obviously shocked them and recalled how Big Brother had indeed sounded ill for the past days. "Kanina nga habang kinakausap niya ako, ubo siya ng ubo (He was coughing from time to time when we talked a while ago)," Mariel shared. But how will the housemates go on inside the house without Big Brother? Well, here's the answer: Big Brother's Big Utol!

That night of the nomination night, just when the housemates were about to sleep, a different wake-up call played for them. Not only were they disoriented as they haven't even gotten to sleep a wink, it also baffled them why they were hearing a tune they had never heard Big Brother used on them. They sleepily trudged from their bedroom to the living and dining area, with Jason asking "Bakit gabi pa? (Why is it still nightime?)." What they didn't know and expect was the new voice that greeted them in the words of "Mga utol!" Thinking it was Big Brother back from the hospital well and sound, everybody jumped happily from they were sitting. However, as Cathy noted, it was not the voice of their beloved Big Brother. The housemates finally felt there was something new coming for them!

And Big Utol did not fail to carry on his prankster-ish attitude. He shocked the housemates with his new way of treatment on them, which was being rough, smart-alecky and bossy. He even redesigned the storage room so he could have a confession room of his own! The walls were covered in black wallpaper and a lamp swayed from the ceiling hovered on the seat where the housemates took their seats when they were called. It somehow resembled the room police use during their interrogations with suspects and it was why the first housemates that were called into the confession room were a tad intimidated!

Paul Jake, Sam and Cathy were the ones who got to talk with Big Utol first. Seeing as their new "brother" seemed nothing similar to their original Big Brother, each of them tried hard in showing that they weren't fazed. But sorry, Big Utol was trickier than they were - he pulled on pranks on them such as making Paul Jake open a bunch of crumpled pieces of paper only to find out a cockroach awaited him, making Sam pour a glass of water on Cathy's face because the latter was sleeping when she shouldn't be and surprising Cathy with her one of her greatest fears: a snake!
