George Calvelo, ABS-CBN News/File." src="https://news-workbench-api.abs-cbn.com/api/upload-raw/2024022610024/93380c3b4864462fbd5e9d19eabc1fac43e850e9cbd645ae53c545523e03c61b.jpg" class="fr-fil fr-dib"/><figcaption>Health workers pass by as a pregnant woman and her companions wait for a ride home just outside the Dr. Jose Fabella Memorial Hospital in Manila on August 19, 2021. <em>George Calvelo, ABS-CBN News/File.</em></figcaption></figure></div><p>MANILA -- The Philippine Statistics Authority on Monday clarified the process parents or children should take for delayed birth registration.<br/> <br/>PSA Civil Register and Management Division Chief, Registration Officer V Emilio Querubin, Jr. explained that the &quot;Timely Registration of Civil Registry Documents&quot; for birth certificates is within 30 days, and any later registration attempts would be determined as delayed registration.<br/> <br/>In case a child was not registered at the time of birth, parents need to submit an &#x27;affidavit of two disinterested persons&#x27; explaining why the documents were not registered on time, as well as any document that will establish the facts of the birth of the child.<br/> <br/>Querubin, Jr. added should a child be born in a hospital, it is the responsibility of the hospital administration to register the child to the PSA.<br/> <br/>Should the hospital administration fail, the responsibility falls on the parents.<br/> <br/>Querubin, Jr. also answered concerns on the name a child should carry depending on their father&#x27;s acknowledgment.</p><ul class="op-related-articles"><li><a href="https://news.abs-cbn.com/entertainment/2024/2/24/angelica-jones-wants-child-to-be-acknowledged-by-father-for-graduation-256">READ: Angelica Jones wants child to be acknowledged by father for graduation</a></li></ul><p><br/>Citing Republic Act No. 9255 or An Act Allowing Illegitimate Children to Use the Surname of Their Father, Amending for the Purpose Article 176 of Executive Order No. 209, Otherwise Known as the &quot;Family Code of the Philippines, Querubin, Jr. said a child 7 to 17 years of age may use their father&#x27;s surname if the child executes an &#x27;Affidavit to Use the Surname of the Father&#x27; (AUSF) with the attestation of their mother.<br/> <br/>Querubin, Jr. also assured the public that once the PSA registers a child, their information is secured in their database and they can provide another copy if the physical certificate is lost.<br/> <br/>&quot;Kahit mawala ang birth certificate [ng bata], once it was registered by the Local Civil Registrar, at naisumite niya ang kopya niya sa PSA, kahit saang pumunta siyang office, halimbawa, kung pupunta siya sa Local Civil Registrar or pupunta siya sa PSA, makakakuha po siya ng kopya,&quot; said Querubin, Jr.<br/> <br/>Querubin, Jr. added individuals who are not registered to the PSA should visit their birth hospital as they are most likely holding a record and proof that someone was born in their facility.</p><div><br/><strong>RELATED VIDEO </strong></div><div><ivs-player data-ivs-key="e8c22c015ca9eafa76f1e3edfa03c603" data-ivs-spid="3001339" data-ivs-vid="2969939"></ivs-player></div><script src="https://player.ivideosmart.com/ivsplayer/v4/dist/js/loader.js"></script>
